What to Expect on Your First Visit
Many new patients are not sure what to expect during their first appointment with a Doctor of Chiropractic (DC).
Paperwork
Before your first visit, we have some paperwork for you to fill out. We will request that you download and complete our patient intake forms located in the ‘New Patient Intake Form’ page.(Click HERE) Alternately, you may contact our office and we will email paperwork to you before your first appointment, along with any outcome assessment forms that are appropriate. So, when you arrive be sure to have your new patient paperwork in hand and be at least 15 min early so we can get you checked in. Upon entering our office our receptionist/office manager will greet you and welcome you as a member of our family.
History & Examination
Dr. Lee D.C. will take your history and give you a thorough physical examination to develop a working diagnosis. Imaging or lab tests (such as MRI, CT scan or X-ray) may be used to confirm a diagnosis.
Review of Findings
Through a process of shared decision-making, you and Dr. Lee will determine if chiropractic services are right for you. As part of this process, Dr. Lee will explain your condition, recommend a treatment plan and review the risks and benefits of all procedures.
